Event Risk Assessment

Use this webform to identify event hazards, evaluate risks, and plan control measures. Consider all phases of the event – set up, the event itself, and tear down – and common risk areas (ie. Crowd control, safe access/egress, fire safety, sever weather) when identifying hazards.

Assess each identified hazards risk level (Low/Medium/High) based on how likely the hazard is and how sever the potential harm could be.  Then detail the measure you will use to control or minimize the risk.  If additional actions or precautions are needed note what they are and who is responsible.
